In select patients, a direct to implant breast reconstruction allows for a mastectomy and implant placement to be performed in a single operation, without the need for placing a tissue expander.

For patients with sufficient healthy breast skin following mastectomy, direct-to-implant breast reconstruction can be a good option. This type of breast reconstruction has gained popularity for its “one-step” approach in completing the mastectomy and breast reconstruction during the same surgery. Upon completion of your mastectomy, your surgeon is able to perform the breast reconstruction with a permanent implant, rather than using a tissue expander. The benefits of direct-to-implant breast reconstruction is that patients awaken from their mastectomy with a well-defined breast shape. 

Key Information/What to Know:

  • Both silicone (more common) and saline implants are used. You will discuss with your surgeon to choose the type of implant best suited to you. 
  • Implants can be placed either over (prepectoral) or under (subpectoral) your pectoralis muscle. 
  • A biologic matrix is often used around the implant to help secure the implant position   
  • Patients come out of mastectomy procedure with well defined breast volume and shape 
  • Minor revision procedures to fine tune shape, symmetry, and size may be needed in future. 

Common Questions

Duration of Tissue Expander Filling: Expansion occurs during office visits at intervals of one to three weeks, with each session lasting approximately one minute. The second reconstruction stage is planned upon reaching the desired volume.

Pain Associated with Tissue Expansion: Most patients experience minimal or no discomfort during the expansion process, as the patient’s skin tightness limits the saline amount added at each visit.

Coordination with Chemotherapy: Expansion sessions can be coordinated with chemotherapy, but the second reconstruction stage is delayed until after completing chemotherapy.
